Our goal is not only to treat your current dental problems but also to educate you on preventing further tooth decay. We would like to be proactive in helping you maintain your oral health at its best for the years to come.

The treatment recommended by Dallas Cosmetic Dentist, Dr. Daftary for your oral health depends on individual condition. As a result, each treatment will be carefully customized after a thorough diagnosis. It is imperative that you continue home care of your teeth and gums in addition to scheduling your routine dental cleaning and professional exams as recommended by Prestonwood Dental.

At this point, let us remind you that the final ownership of your oral health rests on you. The long-term success of our caring treatment greatly depends on how sensitive you are to your oral health.

Treatment Covered by Limited Dental Warranty

Our Obligations

(Written warranty will be provided at the time of your visit)

Your Responsibility

(Failure to keep the prescribed appointments and treatments by our office will void this warranty)

Dental Sealants

We will repair or replace Sealants for a period of 3 years from the placement date.

If decay is present on the chewing surface, the replacement filling will be done at no charge.

You must keep the prescribed regular Hygiene appointments (usually 6 months) within a 21-day grace period for routine professional exams, X-rays, and Cleaning at our office.

Crowns
Bridges
Onlays
Porcelain Veneer

We will replace or repair the same type at no extra charge to you during these 3 years.

If a veneered or onlayed tooth is damaged or decayed to the extent that a crown is necessary, the full fee originally paid for the veneer or onlay will be applied toward the crown. You will only pay the difference.

You must keep the prescribed regular Hygiene appointments (usually 6 months) within a 21-day grace period for routine professional exams, X-rays, and Cleaning at our office.

This does not include accidents, acts of god, and willful damage to the prosthetics, which could affect normal healthy teeth.

We can not predict, if and/or when gum or nerve treatments may be required on the covered tooth. If needed, they would require additional fees.

Composite (Tooth Colored) Fillings.

If the composite restoration fails, we will replace or repair it for a period of 3 years from the placement date.

If the tooth breaks and requires a crown or onlay, we will credit the cost of the filling towards the crown or onlay.

You must keep the prescribed regular Hygiene appointments (usually 6 months) within a 21-day grace period for routine professional exams, X-rays, and Cleaning at our office.

We can not prognosis if and/or when gum or nerve treatments may be required on the covered tooth. If needed, they would require additional fees.

Dentures and Partial Dentures

We will repair dentures and partials for a period of 3 years from the placement date, if a tooth chip, breaks, or flange breaks under normal use.

This warranty will not apply to any accidents and dropping your dentures.

Full upper and lower denture patients must be seen once every 12 months in our office.

Patients with some of their own natural teeth must be seen at the prescribed recall appointments in our office
(Minimum 6 months) for routine professional exams, X-rays, and Cleaning.

DISCLAIMER AND WHAT IS NOT COVERED BY THE LIMITED DENTAL WARRANTY:

  1. Cash refunds for any treatment rendered.
  2. 2. Failure or repairs resulting from the act of god, neglect, abuse, failure of the supportive tooth or tissue structures, improper adjustments, or improper dental hygiene.
  3. Failure to follow periodically (minimum 6 months or 3-6-9-12 months) dental visits as prescribed by our office for a professional exam, X-rays, and cleaning.
  4. Failure to complete necessary dental treatments as prescribed by our office to prevent Gum Infection and Tooth Decay, which has a tendency to spread from tooth to tooth.
  5. Inform our office of the need to change or cancel appointments at least 48 hours prior to your scheduled appointment. Shorter notice of change or cancellations will severely limit our ability to offer another patient their most preferred time.
  6. Incidental or consequential damages, including inconvenience, lost wages, or pain and suffering.
